Why don't you turn it on and see what happens, if your game chugs or screws up, turn it off, if not, keep it on.

It enables open GL i believe rather than direct 3d usage. I would leave it on for most graphic cards. I noticed that my fps was better and also the coloring was nicer.

JO always uses OpenGL, never Direct3D, ever. Some goes with all other Quake III based games.

Enabling GL extensions enables hardware support for some more advanced rendering features. Turning it off forces it to be calculated with software, which is slower. You want this on.
