Has anyone ever wondered what the Land of the Dead looks like from "above"? Is there any map available, fan-made or otherwise...

Right here.

As for me, I'd like to leave it to my imagination, and think of it as a MUCH bigger place with Much cooler place to go.

Nice map! Is it fan-made, and are also the locations we've never heard of, like Iago mines and Rim Tundra?

I think those locations are fan-made as well. The pics are certainly not from the game.

As far as I know, some of the content and locations maybe fan made, either a reference to fan fiction or other, have to read more fan fiction.

Yeah, that map is entirely fan made (as already pointed out) and the author added some fictional locations to flesh it out a little. The only ones that were actually in the game you'll recognise immediately. :)
Like Du Man though, my personal interpretation of the Land of the Dead is a much vaster place, although I think the fan map is probably accurate with regards to things like how the edges of it look (we obviously know it's flat, not a sphere), and El Marrow and the gate being on opposite ends.
