Have you tried turning off the "hardware acceleration" option from the menu?

I have exactly the same problem!

I installed the patch and followed all procedures shown in previous threads... It is driving me crazy!!

Does anyone have a simple step by step way of doing it?

Ok, the problem is your monitor, it is too big for the game (i had the same problem with a 22"). The only solution is to play the game windowed

I'm guessing you have an Nvidia 8xxx series card or newer, that being the case, that's what the game looks like. There is no known fix.
